Community Care Team

The Community Connecting Team (CCT) is part of Adult Social Services and offer our service to adults of all ages. We support individuals whom may have a learning disability, experience mental ill health, be vulnerable and/or are socially isolated, in accessing activities in the community.

We help individuals build confidence, self-esteem and to develop skills so that they can become more independent. Our activities also give the opportunity to make friends and meet new people. We offer a range of activities and opportunities around Neath Port Talbot, both indoors and out and welcome enquires about our service.
